Did you know you can access GP services from your computer, tablet or mobile phone with Patient Online?  You can book GP appointments, view a partial summary of your health record which includes Medication, Allergies and Immunisation history or renew prescriptions via the clinical system.  To register for this service please come to the surgery reception for further details.  You will need 2 forms of identification (one needs to be photo ID such as a passport or driver's licence, the other a utility bill to confirm your residence) to access this service. Registering at the practice

Patients wishing to register at the practice should complete a registration form obtainable from the main reception desk. You will need to provide a form of photographic ID and a utility bill to prove your residence is within our agreed practice boundary. Once completed and returned your details will be entered on our system and the process of requesting your notes from your previous practice will commence.   It may take a few weeks before your notes arrive at the practice and so you will be advised to use our Health Pod which will capture your blood pressure, weight and height which then calculates your BMI.  Medication requests for new patients will need to be discussed with a GP within a telephone consultation.
