Registering at the practice
Patients wishing to register at the practice should complete a registration form obtainable from the main reception desk. You will need to provide a form of photographic ID and a utility bill to prove your residence is within our agreed practice boundary. Once completed and returned your details will be entered on our system and the process of requesting your notes from your previous practice will commence. It may take a few weeks before your notes arrive at the practice and so you will be advised to use our Health Pod which will capture your blood pressure, weight and height which then calculates your BMI. Medication requests for new patients will need to be discussed with a GP within a telephone consultation.

As of 2021 we do not accept requests for registration from patients outside of the practice boundary area or accept a change of address to outside of the boundary area.
