Registering at the practice
Patients wishing to register at the practice should complete a registration form obtainable from the main reception desk. Once completed and returned your details will be entered on our system and the process of requesting your notes from your previous practice will commence. It may take a few weeks before your notes arrive at the practice and so you will be offered a new patient check with the practice nurse, who will go over any previous medical history and cover any basic health checks, such as blood pressure and weight.
Before you attend for the new patient check, it would be very helpful if you completed the online medical questionnaire (below) so that this information will be readily availble prior to your appointment with the nurse. Please note this form is NOT a registration form for joining the practice.
